These Terms of Service (“Terms”) govern your use of the website at dormahealth.com (the “Site”) and the at-home sleep apnea testing service provided by Dorma Health, Inc. (the “Service”). By using the Site or the Service, you agree to these Terms.
You must be 18 years or older and shipping to an address in the 50 U.S. states or Washington, D.C. to order the Service. By placing an order you represent that this is true and that the information you provide is accurate.
Dorma is not itself a medical practice. The clinical care components of the Service (screening order, interpretation of the home sleep study, and any prescriptions) are provided by licensed physicians on the Dorma network, who are independent professionals exercising their own clinical judgment.
The Service is not for emergency use. If you or someone with you is experiencing a medical emergency, call 911 or go to the nearest emergency department.
The Dorma sleep test device is loaned to you for the duration of your recording and must be returned in the prepaid mailer provided. If the device is not returned within 30 days of delivery, you authorize Dorma to charge a non-return fee for the cost of the device. If you do not medically qualify for the at-home test after physician review, the Service fee is refunded in full.
The Service is currently sold for a flat fee of $189 per test, payable by major credit card, debit card, or eligible HSA/FSA card. We do not bill insurance.
The Site, including its text, graphics, logos, and software, is owned by Dorma Health, Inc. or its licensors. You may not copy, modify, or distribute it except as allowed by these Terms.
The Site and the Service are provided “as is” and “as available.” To the maximum extent permitted by law, Dorma disclaims all warranties, express or implied, including warranties of merchantability, fitness for a particular purpose, and non-infringement.
To the maximum extent permitted by law, Dorma will not be liable for any indirect, incidental, consequential, special, or exemplary damages, and Dorma’s total liability for any claim related to the Site or the Service will not exceed the amount you paid for the Service in the 12 months before the claim.
We may update these Terms. If we make material changes, we will let you know by email or with a notice on the Site.
